Millbrook Employment and Training will be hosting a Mobile SIN Clinic:
-Request SIN for the first time.
-Obtain confirmation of SIN Number
-Canada Pension
-Old Age Security
-Guaranteed Income Supplement
-Canada Disability Benefit
-Canada Dental Care Plan
Documents needed for service are:
-Long Form Birth Certificate (**Original Copy not a Photocopy**)
-Photo ID ( Status Card, Licence, or ID)
