Medical Transportation Reimbursement Form - Atlantic Region (April 2023)

NIHB Medical Transportation Reimbursement:  If you have an upcoming medical appointment and need transportation to it, and you are a registered status FN person, please contact NIHB (1-800-565-3294) and press the option for Medical Transportation to speak to someone about your upcoming trip.  That person will complete the reimbursement form with you and send it to you by email or mail for you to sign.  You must include a letter confirming that you have an upcoming appointment or that you already had the appointment, signed by someone at the place you will or have had the appointment.  You can then fax, scan or take a picture of the signed documents and send that back to the contact at NIHB.  Please do not leave this to the last minute as NIHB will need time to process your request.
