Reporting to the Director of Health & Social Services, you will work within a collaborative, interdisciplinary team that supports the full spectrum of health, including physical, emotional, mental, and spiritual well-being. This role is suited for someone who looks beyond symptoms to understand the full story of each patient. You build relationships with empathy, respect, and integrity, creating a space where patients feel heard and supported. You are committed to culturally safe care and approach your work with curiosity, openness, and a mindset of continuous learning. In a dynamic, community-based environment, you will work closely with physicians and a diverse care team, including community health, mental health and addictions, home care, and nutrition services, contributing to coordinated, person- and family centred care.
