2026 Millbrook Elections: Mail-in Ballots

Requesting a Mail-in Ballot

People living on or off-reserve who may want to request a mail-in ballot.

  • Members that are unable to physically come to the polling station on Election Day.

  • Members that may be incarcerated.

  • Members that may be in a health care facility.

  • Members that may be at school can get mail.

  • Members that may be away on election day.

  • Senior band members.

  • Members that may have moved and have not updated their address with Electoral Office or Band (please state it’s for elections).

We cannot accept a third-party request for ballots. It must come from the person who would like a mail in ballot. All requests must include name, address and band number.

Important Mailing Instructions

Please make sure that you complete the mail-in Voter Declaration Form.

  • The voters declaration form must be witness when you fill it out and they in turn must sign the voter declaration form.

  • The witness can be anyone 18 years and older.

  • It is very important that this document is filled out correctly. You will see an example of one side of the declaration form. If there is no witness filled out on the voter’s declaration form, the ballot will not be accepted.

  • Place your ballot in the “ballot only” envelope. Do not place your voters declaration form in with your ballot.

  • Place your voters declaration form in the return envelope along with your “ballot only” envelope.
